Fairview Maple Grove Medical Center now offers a diabetes education and self management program specially designed for patients who are:
- New to diabetes.
- Children or adolescents with diabetes.
- Beginning insulin or diabetes medication.
- Desiring to lose weight.
- Pregnant or planning a pregnancy.
- Requiring adjustment of insulin, oral medication, or diet and exercise plans.
- Seeking better blood sugar control through intensive medication management, carbohydrate counting, or insulin pumps.
- In need of a diabetes update or have not seen a diabetes educator in the past.
Program Services:
- Comprehensive self-management training for children and adults.
- Personalized instruction by two experienced Certified Diabetes Educator, one who is also a clinical nurse specialist and the other, a dietitian.
- Ongoing follow-up and support after your education is complete.

A referral is required from your provider for participation in this program.
