Fairview Maple Grove Medical Center now offers a diabetes education and self management program specially designed for patients who are:

• New to diabetes.
• Children or adolescents with diabetes.
• Beginning insulin or diabetes medication.
• Desiring to lose weight.
• Pregnant or planning a pregnancy.
• Requiring adjustment of insulin, oral medication, or diet and exercise plans.
• Seeking better blood sugar control through intensive medication management, carbohydrate counting, or insulin pumps.
• In need of a diabetes update or have not seen a diabetes educator in the past.

Program Services:

• Comprehensive self-management training for children and adults.
• Personalized instruction by two experienced Certified Diabetes Educator, one who is also a clinical nurse specialist and the other, a dietitian.
• Ongoing follow-up and support after your education is complete.

A referral is required from your provider for participation in this program.
